 The best way I can think of avoiding these kinds of conflicts is to add a
 short hash to filenames of intermediate image sizes to ensure that this is
 unique. The main question would be whether it would be whether we needed
 to build that hash from something that is unchanging (e.g. the original
 file name) rather than something that could change (e.g., current server
 time) so that regenerating images would result in consistent image file
 names as ones already used in post content.
