 I think this issue should be split into two separate cases:
 1. Related to a wrong image format (CMYK), when the image is never
 correctly uploaded (some cases of too big images could match this category
 as well)
 2. Related to random errors, which cannot be reliably reproduced, related
 probably to server and PHP configuration, timeouts and other errors,
 leaving garbage in the database.

 And it should be clearly stated that switching to GD is not a fix, it's
 just an ugly workaround with some other consequences (like quality).
