 You're absolutely right @westonruter, I did a fresh install on another
 domain with an identical setup, everything worked fine.

 After digging more, I tried clearing cookies on my browser and it fixed
 the problem.  Apparently some of the back-and-forth configuration options
 I played with during initial setup (subfolder vs subdomain, adding domain
 mapping, etc) left a bad cookie in place, which caused the UUID issue.

 All is well now.  Thanks for the time you spent looking into this!
