 In 21583.10.diff I worked on:

 - Need to hide the cog on pages where there are no screen options (e.g
 media-new.php).
 - Changing "Number of columns" doesn't do anything.
 - When your browser height is less than the height of the dropdown, there
 is no way to see all of the options.

 @meliko - let me know if you want to work together on "Items per page" on
 edit.php and other list pages, needs some CSS love.
