 Running down hakre's list, as handled in [13343]:

 2-8, 11, 13-18, 20 handled. 1, 9, 10, 12, 19 not. Additional notes:

 Had to modify the logic for error_log.

 13 and 17 were handled, but with examples that doesn't assume %d, as that
 only works when you're dealing with a core table that is set up in
 wpdb::$field_types. Additional comments added to make note of that.

 Further suggestion 1 was handled.
