[theme-reviewers] Theme ads widget/ code/custom contact form using template

@devcorn: You have pointed out in the trac ticket that all three things are
required.

*Required*

   - Contact page, Ad widget and adding scripts in head/body are plugin
   territory


Please distinguish between required and recommended items. No offense ;)
