[theme-reviewers] Theme ads widget/ code/custom contact form using template

[Stuart Wider]

Users need to be able to select which type of page template it will show on
(home, page, post etc) so basically just has a few extra tick boxes.

My theme HeatMap AdAptive is specially designed for experimenting with ad
placement so if it didnt have ad widget functionality built in it would be
inconvenient and confusing for the user.

The basic concept has been very popular over at my site HeatMapTheme.com
for the last four years, and was very popular when I released the first
ever version of HeatMap Theme on WordPress.org.
