[theme-reviewers] Codex: Theme Review page

Looks great!

My only comment regards the by-file listing for functions, tags, and hooks:
I had it laid out that way to facilitate stepping through the Theme files
during the review: functions.php, header.php, index.php, comments.php,
sidebar.php, footer.php.

If we could organize the list such that it sorts by
required/recommended/optional, and still facilitates a single-pass Theme
step-through, that would be awesome. I'll take a look (later).

The next thing on my to-do list was to add Codex links, so I'll try to clean
that up, also, when I get the chance.
