I created a simple admin page using your template with a form with an
action of "options.php".  Clicking the button gives:

Cannot load lm.php?updated=true

with a URL of:

http://path/to/wp-admin/admin.php?page=lm.php?updated=true

Should that second "?" be a "&" ?
