Right now I'm using:

a) comprehensive blacklist
b) checking MX records of e-mail address domains (no
blah at awefawefawefwef.com)
c) checking all URIs against public spam domain blacklists
d) checking IP against IP blacklist (open proxy blacklist)
e) Links limit set at 1
f) if comment goes into moderation, user is redirected to a CAPTCHA
where they can approve the comment themselves by proving their humanity.

Many of those things were rolled into Dave's Spam Karma, which is
probably the best option for most people.
