You have to set it in the root index.php only after upgrading from
1.2.2.  It's a one time deal and done only when upgrading.  Also, it
serves as a marker for the root index.php, which comes in handy and
fixes some problems.

To be clear, this is not set in the theme's index.php, but in the root
index.php.
