 Could you provide the steps to reproduce the issue on a clean install?
 Otherwise, I don't think there is any actionable information here yet to
 continue.

 Yeah, that happens once in a while when Trac is under heavy load, but I
 think it's unfortunately a bit outside of WordPress Core team's control.
