 ready for review. It includes unit tests for:

 - `Theme_Upgrader::install`
 - `Theme_Upgrader::upgrade`
 - `Theme_Upgrader::bulk_upgrade`
 - `Plugin_Upgrader::install`
 - `Plugin_Upgrader::upgrade`
 - `Plugin_Upgrader::bulk_upgrade`

 Hey @dd32, Andy and I were talking about whether to use `time()` or
 `microtime()`. Do you have a preference for the `time_taken` in the error
 data?
