Changes (by ianmjones):

 * Attachment "53668.4.diff" added.

 More robust handling of versioned files, uppercase extensions including on
 pre-existing files, and re-analysis of of original upload type previously
 tested alternate types in response to filename versioning. Inspired by
 @azaozz's alternate PR, and includes his changes for tests and
 wp_get_default_mime_type_extension.
