 1. i do make sure that the video is physically exists in my server and the
 URL matches the path.
 2. Checked my .htaccess and see if there's something there that's blocking
 access to *.mp4 files. nothing there.

 i owned both vps and dedicated server where the file and the website is
 hosted. so the problem is in new wordpress update i guess.
