Comment:

 This ticket was discussed during today's accessibility bug-scrub: agreed
 to split it into smaller tickets. Some of the simpler parts could be
 doable for 5.5. For example:
 - fixing alt text when inserting images in the editor
 - deciding how alt text should work in featured images and implementing it
