 Regarding:

 Unfortunately it didn't happen for lack of resources and prioritization.
 This ticket addresses only 2 instances: in the Media Library and in the
 Media modal. A follow-up ticket should be created to audit al the other
 instances in core. Changing the type of this ticket back to "bug".
