 Hi @warengonzaga,

 Based on the screenshots, I do not think that this is an issue with
 WordPress Core. No similar code is included in Core. I recommend opening a
 support ticket in the [http://wordpress.org/support/forums WordPress.org
 Support Forums] to debug further.

 Also, in the future, please upload screenshots directly to tickets as
 attachments. External images often go missing or are removed over time and
 it makes it difficult for future contributors to understand past issues.
