 In our case, we had Wordfence installed but not active on the site in
 question.

 The server error log had:

 }}}

 Uninstalling Wordfence resolved the error for us.

 Perhaps the uninstall edited .user.ini & .htaccess to prevent the
 Wordfence firewall from running.
