 The new text and checkbox fields for comments when users are not logged in
 does not seem to appear on custom theme (Forte). Probably theme related,
 but 3rd party plugins with checkbox "send mail when comments are replied"
 styles works fine (also tried to deactivate this to see if conflicting).
