 Thanks for the ticket @loranrendel! I think that this might be a duplicate
 of #32315. It deals with the same general issue of `$wpdb->insert()`
 failing without an error message. If others agree, we can close this as a
 duplicate, since there is already some discussion and a patch on that
 ticket.
