 Ever since upgrading to 4.9.4, I've noticed a bug the media grid uploader.
 When adding a new media item, the new item placeholder (with progress bar)
 is inserted at the second to last media item.  Then at the end of the
 upload, the item is correctly placed at the top of the list.  For a user
 like me with hundreds of media items, it can seem like my upload has
 failed because I can't see the pending upload without scrolling down to
 the bottom of the list.  Here's some screen shots from my dev site with no
 plugins activated.

 New Uploads Appear at the second from last position:

 During upload, an image that has completed uploading will be added to the
 first of the grid while maintaining it's orignal "pending upload"
 position:

 After all images have uploaded, the images are correctly placed at the top
 of the grid
