 1) It could be readme.txt or readme.md

 2) I suppose it could start as:

 }}}

 just like

 == Changelog ==
 }}}

 3) Below that standard paragraphs or is any further 'formatting' needed?

 4) Versioning would be done by increasing the version of the plugin (?)
