 Patch refreshed!

 I've left out the style-guide cleanup of the rest of the plugin-editor and
 theme-editor files, since it made it hard to see what changed.

 I've adjusted the folder lines to only show for subfolders, and also added
 mobile styles that I had forgotten in the last patch. Also, the file list
 is now the same size as the code editor area, with scrollbars if needed.

 I've tested on php5.6 and php7.1

 Can someone confirm that it's OK on php 5.2 & 5.3?
