 Refreshed patch. I'd say to take care of `maxlength` in #40610.

 Spotted a few more things, most notably: `$detach_url` in the media list
 table needs to be escaped. Other things:
 - 1 more `permalink` rel attribute in Multisite
 - missing span in `wp-admin/includes/template.php` (in the "Attach media"
 modal)
 - let's make that table row in the Users screen use a td instead of a th,
 there's no need for a table header there
