 @lukecavanagh The termmeta table has similar indexes to the other metadata
 tables. See https://core.trac.wordpress.org/browser/tags/4.6.1/src/wp-
 admin/includes/schema.php?marks=61,62#L54. The codex article you've linked
 is out of date. Thanks for the ticket!
