 @tgelles - Thanks for sharing the details of how you resolved this issue
 for your install, this may help other users who have a similar issue.

 Since this was was a configuration issue with your `.htaccess` file and
 not a WordPress issue I am going to close this ticket. If you continue to
 see the problem and think they are related to WordPress itself, you can
 re-open the ticket.
