Comment:

 In [attachment:31050.diff], refreshed patch, made a few whitespace
 changes, and added a couple of things we'll want to consider before a
 commit in TODOs.

 This should do for testing, but does not resolve any of the issues found
 previously with broken thumbnails being generated.

 Please test this with your PDFs and please attach any test PDFs you find
 that break the functionality!
