 Just stepping in to report that I am also seeing the same problem on a
 site running 4.5.2.

 I've tried all the suggested solutions including the diff patch,
 functions.php alterations and the .htaccess addition. Tried switching to
 Twenty Fifteen theme to rule out the theme but still no luck.

 Hosting provider : 1and1
 Theme : Enfold based child theme
 PHP Version 7.0.7
 memory_limit    512M
 max_execution_time      360
 post_max_size   64M

 The file I'm using to test at the moment is 1,481KB, 4500 x 3000 pixels.

 I can confirm that the file gets uploaded into the uploads folder but no
 size variations are produced.

 Please find a fix soon as my daughter is giving me grief after I just
 moved her blog from Blogger to WordPress. :-)
