Thank you everyone for working on this. I've experienced the same issue on
2 client sites now. Both running on WordPress 4.5.2 and on the same host
(Site5). In our case the functions.php fix suggested by @smashingjay
solved the problem in both cases.

This wasn't an issue on these 2 sites before. When initially
troubleshooting I turned off all plugins, which made the image uploads
work again. But then re-enabling plugins one by one I couldn't isolate a
single plugin as causing a conflict. It was more that after 3 or 4 plugins
were active again, the issue reappeared. But it didn't seem to matter
which plugins.
Other client sites on the same server with very similar setups are not
having this problem. The common thing with these 2 sites was that they are
both running a child theme. I'll check some of our other sites to see if
that might have anything to do with it and report back here.
