 @joemcgill I have ran into this problem recently but I am not sure if it
 was on a site running 4.5 or a version prior.  However I have no idea what
 causes it.   I thought it had something to do with clicking somewhere
 before the uploader has captured the drop of the images but I can't
 consistently replicate the issue.  So I am really not sure.
