 In [https://core.trac.wordpress.org/attachment/ticket/4332/4332.4.diff
 4332.4.diff], I replaced the manual "Back" link and used the `back_link`
 arg in `wp_die()` to generate the same output. Also set a title for wp-
 comments-post.php when there is an error to `Comment Submission Failure`
 (open to better suggestions here as well).
