Comment:

 Thanks for the patch @sanket.parmar but the issue is more complex than
 this.

 * The link text only needs to change for unapproved comments. The
 behaviour of replying to an approved comment doesn't change.
 * The link text needs to be aware of AJAX actions. For an unapproved
 comment that gets approved, the link text needs to change from `Approve
 and Reply` to `Reply`, and vice versa. This will need some JavaScript
 work.
