 In our case it was a Vimeo plugin but I gather from other comments that a
 variety of plugins and themes cause the same problem due to incorrect
 coding. I think if there's a Wordpress core bug here, it's that WP doesn't
 log the plugin that caused the problem, making it difficult to diagnose
 and fix. Maybe it's not possible for WP core to identify which plugins
 cause it?
