 [attachment:32417.3.diff] relocates the title field to match other core
 widgets and renames the `WP_Widget_Media` class file to fit the standard
 naming convention: `class-wp-widget-*.php`.

 Noticed some JS errors when attempting to add media to the widget that
 would need to be resolved.
