 In the Edit Media screen, the "Description" and "File URL" labels should
 be properly associated with the respective form elements. About the
 latter, the label should be correctly associated even if the input field
 is "readonly". Currently, when focused they're announced by screen readers
 as "edit multi line" and "edit readonly" without any feedback about their
 purpose.
