 The latest
 [https://core.trac.wordpress.org/attachment/ticket/31528/31528.10.diff
 patch] contains the original, but now reverted, changes along with a fix.
 Instead of going the ajax route, I went with moving the form generation to
 be dependent on the need for the form and checking for that instead of
 doing it inside the script loader.

 If you want to see just change from the previous code,
