 In general this is working well for me, but I've come across a problem
 with this and W3 Total Cache.  If you want to upload the wp-includes files
 to a CDN, W3TC (unaware of the rewrite rule) uploads the files to the CDN
 with the subdirectory in the path, but rewrites them on the site without
 the subdirectory (as there is no path in the siteurl, WP enqueues them
 without the subdirectory, and W3TC just rewrites what's requested).

 Has anybody else encountered this and found a solution?  I've also asked
 in the support forums, but thought here might be more productive.
