 I've modified the patch to remove the <button> element.  I also included
 theme-install.php as well.

 As far as separate tickets, I don't have a strong opinion either way.  I
 don't mind going into each screen and making the needed changes.  At first
 glance it looks like the themes screen was the only one that involved the
 javascript. The others were just a matter of moving the code outside the
 <h2> tag.
