 Add media should probably not be hidden, unless there are buttons added by
 plugins. I would consider the screenshot an edge case, although apparently
 something WordPress.com is running into... not sure that this is the best
 place for plugins to be shoving all of these buttons anyway. A lot of that
 functionally could logically be integrated into the media modal, or the
 concept of "content blocks" could be explored further.
