 Yes, adding another wrapper had occurred to me, as well. There are *so
 many* wrappers already, I was reluctant...but I don't see another
 reasonable alternative. Z-index stacking contexts are a painful rabbit
 hole.

 I think yes, it would make sense to alter the placement of skip to toolbar
 conditionally; I'll go ahead and integrate that into this patch, since
 it's fairly closely related along with everything else we're doing.
