Comment:

 Yes, it's intended - when an image is inserted into content, it is
 effectively separated from its attachment and becomes a part of that
 post's content instead. The fields that remain do not get synced back to
 the attachment in the media library but rather only apply to that instance
 of image content, and so other fields that don't display in the content
 aren't present. It's a little bit of a mind-bend, but since images can be
 inserted from other sources, there is a separation there.
