 Hi Eddy, welcome to Trac!

 I'm unable to reproduce this issue on a clean multisite install running
 4.1.1. Comment recipient emails are being sent as expected.

 It sounds like maybe you have a plugin that's hooking to the comment
 recipients hook and modifying who is getting notified. I'd suggest
 deactivating all of your plugins and trying again.
