 The process described in http://codex.wordpress.org/Create_A_Network can
 never work because the document fails to include the requirement to
 {{{define( 'MULTISITE', true );}}}
 in wp-config.php, nor is that requirement ever displayed during the
 Multisite Setup process.
 Without it, the user sees only (the very misleading) error message 'Error
 establishing database connection' and is required to search the forums for
 the solution.
 Please update http://codex.wordpress.org/Create_A_Network to add this
 critical requirement.
