 * Adds unit tests for `get_avatar()` and `get_avatar_url()`
 * Fixes `get_avatar_url()` failing if it's passed the array version of a
 user/post/comment object
 * Fixes `get_avatar_url()`'s `$args` parameter not being passed as a
 reference
 * Fixes `get_avatar_url()` returning a broken URL if it couldn't figure
 out the email hash
 * Tweaks the docs just a little more
