 This can also be improved: can insert both `<pre><code>` at the same time.
 Or perhaps can have another TinyMCE command and shortcut for that.

 A bit of a downside is that if the user selects multiple paragraphs, all
 will be converted to `<pre><code>...</code></pre>`. Of course that can be
 undone or toggled back easily.
