Comment:

 I just got an email from a desperate guy trying to log in to his freshly
 installed WordPress site (started over four times, same error with login,
 and later confirmed by me).

 I have no idea what the root cause may be, but I hope it can be seriously
 investigated to see if there is something WordPress can improve to help
 here. Something is not right when this happens.

 I will report back as soon as I have some more information.
