 [attachment:27366-07.patch 27366-07.patch] refactors how the tinymce DOM
 is updated when a change is made via the modal. Instead of replacing DOM
 nodes, the patch manipulates the nodes, which means that updating an
 image, caption, or image link doesn't stomp on any custom attributes that
 the user may have added via the Text editor or some other mechanism.
