will get used instead and is actually stored as post meta instead of

Helen, can I ask why that is, as it doesn't seem sensible to me.
That means you could end up with nonsense strings getting into the alt
attribute - and thus to screen readers. If someone is not putting
alternate text in they either don't understand what the field is for or
they are purposefully leaving it blank. If then the title - which may be
the default filename initially gets put into the alt I think that's not
good.
