Comment:

 I think this is a little misleading because of the existing post_title
 field that is being used here - if you don't specify different alt text,
 it will use the title for the alt text and does not output a title
 attribute by default. If you specify a different value for alt text, it
 will get used instead and is actually stored as post meta instead of
 post_title.
